One copy of the Building Permit Application completely filled out
Contractor's drawings and list of materials (if applicable)
One opy of Site Plan, showing placement of house or building, lot line distances, driveway, well and septic sytem locations
Certificate of Insurance and Worker's Compensation for contractor(s), or affidavit of exemption
Return application and accompaning documents to the Code Enforcement Office for review and approval
